from the "valid from" date on your first I-797 form.
No. It starts from the first entry with the H1 visa, or from the date of the
I-94 that is attached to the I-797 approval notice, if it was a change of status.

I don't know if any of your assumptions are warranted, but that was my case.
I got mine on 11/01/00 (the "valid from" date) and after the 3 year renewal
the expiration date is 10/31/06. I have no idea of the circumstance of the
OP, however.

A visa does not say anything. A person could have an H1 visa and not enter the
US for the whole validity of the visa.
The 6 years are 6 years in H1 status. And a person by definition only has a
status while in the US.
Now, CIS may indeed extend a visa based on the original visa date, but
additional time spent abroad, either before the first entry, or time spent
abroad after the first entry, e.g., vacation time, can be recaptured.
Check with your lawyer.
